Practice Survey for Professionals
For purposes of this survey, a collaborative case is a matter where, at a minimum, (1) each client had an attorney, and (2) the parties signed a participation agreement that contained a disqualification clause prohibiting the attorneys from engaging in litigation on behalf of the clients. A case does not become “collaborative” until both parties and both attorneys have signed the participation agreement.
Only 1 survey form should be submitted for each case. By completing this form, you certify that you are the professional in your case designated to submit this survey.
If using the survey as a debriefing tool, prior to the debriefing each professional should have on hand the amount he or she billed and the amount of hours worked on the case, including amounts and hours expected through final hearing (if any).
This survey should take about 10-15 minutes to complete.
